Do You Feel Burnt Out? You Might Be Running on Broken Hormones.

It started for me with a pattern I couldn't explain. Wired at 10pm, completely useless at 9am. Crashing somewhere around 2pm like clockwork. Craving sugar like it was oxygen. Snapping at things that would have rolled off me a year earlier. Not hungry in the morning, then absolutely feral by 3pm.


I chalked it up to stress. Work was relentless. My sleep was inconsistent. I was on my third double espresso before noon most days. It made sense, in a vague way, that I would feel like this.


What I didn't know — and what most of the wellness content I was consuming wasn't telling me accurately — was that "stress" isn't just a feeling. It has a hormone. That hormone has a rhythm. And when that rhythm breaks down, it takes everything else with it.


That hormone is cortisol. And if any of that list above sounds like your average Tuesday, your cortisol rhythm might be the thing worth looking at.

The Orchestra Analogy That Actually Made This Click For Me


Dr. Jaclyn Tolentino, a board-certified family physician and functional medicine doctor at Love.Life, describes it like this: your hormones are like an orchestra — each section playing a part in your energy, metabolism, mood, and sleep. When one instrument is out of tune (usually cortisol), the entire symphony starts to sound off.


That framing hit differently for me, because I'd been thinking about cortisol as a single problem with a single fix. What the research actually shows is that cortisol is more like the conductor — and when the conductor loses the beat, it doesn't matter how good the violin section is.


So here's the actual science of what cortisol is supposed to do, and what happens when it stops doing it correctly.



What Cortisol Is Actually For (Before We Talk About What Goes Wrong)


Cortisol is a glucocorticoid hormone produced by your adrenal glands. It's regulated by the hypothalamic-pituitary-adrenal (HPA) axis — the communication system between your brain and your adrenal glands that essentially decides how much cortisol you need and when.[¹]


In a healthy pattern, cortisol follows a predictable daily rhythm. It bottoms out around midnight while you sleep, starts climbing in the early morning hours before you wake up, and then surges sharply — by 50 to 75% — in the 30 to 45 minutes after you open your eyes.

This surge is called the Cortisol Awakening Response (CAR), and it is your body's way of booting up. It mobilizes energy, primes your immune system, sharpens your focus, and prepares every downstream system to function for the day. After the CAR peaks, cortisol is supposed to decline steadily throughout the afternoon and evening, hitting its lowest point again by late night so your body can shift into rest and repair mode.[²]


This is the rhythm that makes you feel like a human being.

When it works. Which, for a lot of us, it doesn't.

What Cortisol Dysregulation Actually Looks Like


Cortisol dysregulation doesn't mean your cortisol is broken. It means the rhythm is disrupted — the peaks and valleys are happening at the wrong times, in the wrong amounts, or both.


The most common pattern in chronically stressed adults is a blunted morning CAR paired with elevated evening cortisol. Meaning: the surge that's supposed to help you start your day comes in weak, leaving you foggy and exhausted at 9am, while the drop that's supposed to help you wind down doesn't happen, leaving you wired and restless at 10pm.[³]


Sound familiar? There's a reason "tired but can't sleep" has become basically a personality type for women in their 30s and 40s.

Here's what that looks like in the body day-to-day:


Wired at 10pm, exhausted at 9am. The circadian inversion. Your energy is coming at the wrong end of the day because your cortisol is peaking when it should be dropping and blunting when it should be spiking.


Afternoon energy crash. If your morning CAR is weak, you're running on fumes by midday. The crash isn't a willpower problem. It's your system running out of fuel that should have been loaded in the morning.


Craving salt or sugar constantly. Cortisol is directly involved in blood sugar regulation. When it's dysregulated, your blood sugar becomes less stable, and your brain sends you after quick energy sources — sugar and refined carbs — to compensate.[⁴]


Brain fog that lifts at night. Again: wrong timing on the rhythm. Your sharpest window is shifted to the evening when cortisol is still elevated, instead of the morning when it's supposed to peak.


Snapping at small things. Chronically elevated cortisol is directly linked to mood instability. The research on this is consistent — HPA axis hyperactivation increases threat-perception and emotional reactivity.[⁵]


Not hungry in the morning. Cortisol helps regulate appetite signals, and a blunted morning CAR often correlates with suppressed hunger in the first part of the day, followed by larger appetite signals later — which wrecks your metabolic rhythm on top of everything else.



The Part That's Specific to Women (That Most Research Ignores)


Here's something that took me too long to learn: women's cortisol doesn't behave the same way throughout the month. It shifts with the menstrual cycle, and most of the mainstream research on cortisol was conducted on men or didn't control for cycle phase, which means a significant portion of what you've read about cortisol regulation probably wasn't written with your biology specifically in mind.


A meta-analysis published in Frontiers in Endocrinology analyzed data from 778 women across multiple high-quality studies and found that cortisol levels are measurably higher in the follicular phase (the first half of your cycle) than in the luteal phase (the second half, after ovulation).[⁶] A separate PubMed meta-analysis covering 28 studies confirmed the same pattern.[⁷]


Here's where it gets practically useful: women in their luteal phase show a higher cortisol response to acute stressors than women in their follicular phase — meaning the same stressor hits harder hormonally in week three of your cycle than it does in week one.[⁸]

What feels like burnout or inexplicable overwhelm in the lead-up to your period is often a real, measurable physiological phenomenon.

It's not in your head. It's in your luteal phase cortisol response.


This also means that stress management strategies aren't equally effective throughout the month. Knowing where you are in your cycle is actually a meaningful variable when it comes to how your cortisol system responds to and recovers from stress.



The Belly Fat Question: Let's Set the Record Straight


I want to address this one directly because the TikTok and Instagram version of this story is running way ahead of the actual science — and in some cases is being used to sell things that don't work.


The claim: cortisol causes belly fat. True or false?


The honest answer: it's more complicated than a simple yes or no, and anyone selling you a quick fix based on this connection is either oversimplifying or misleading you.


Here's what the research actually shows: cortisol, when chronically elevated, tends to redistribute fat toward the abdominal area and can increase the number of fat cells — particularly visceral fat, the deep metabolically active fat that surrounds your organs.

Visceral fat is meaningfully different from subcutaneous fat (the fat under your skin). Unlike subcutaneous fat, which is relatively inert, visceral fat secretes inflammatory cytokines including IL-6 and TNF-alpha that worsen insulin resistance, raise cardiovascular disease risk, and further disrupt hormonal signaling. So the cortisol-visceral fat connection is real and documented.


There's also a PubMed study measuring urinary cortisol output in premenopausal women that found higher cortisol secretion in women with elevated waist-to-hip ratios, specifically those with higher visceral fat accumulation — suggesting a bidirectional relationship where visceral fat also dysregulates the HPA axis further.[⁹]


But — and this is the part that matters — framing cortisol management as a path to cosmetic changes like a flat stomach reduces a complex health process to an aesthetic issue. The appearance-related concerns supposedly solved by cortisol regulation closely align with western beauty ideals that are especially gendered, targeting women with an ever-elusive hourglass figure.


The real concern with visceral fat is metabolic health, cardiovascular risk, and insulin resistance — not whether you look a certain way in jeans. And the interventions that actually address cortisol-driven visceral fat are not supplements or detox teas. A 2021 meta-analysis in Sports Medicine covering 23 trials found that moderate-intensity continuous training reduced visceral adipose tissue area by an average of 6.9% over 12 weeks — and daily HIIT, conversely, keeps cortisol chronically elevated in women who are already stressed and often worsens rather than improves the picture.


So yes, cortisol and abdominal fat are connected by real physiology. No, "cortisol belly" is not something you fix with a $60 supplement or a 30-day detox program. The interventions are the boring unsexy ones: sleep, stress reduction, moderate exercise, and time.



On Supplements: What the Research Actually Says


This section exists because the supplement industry has fully taken over the cortisol conversation online and I want to be direct about what the evidence does and doesn't support.


The first thing to understand is that cortisol is not your enemy. It is an essential hormone. You need it to wake up, regulate your immune response, manage blood sugar, and respond to genuine stress.

The goal is not to eliminate cortisol — it's to restore the rhythm. Any product or protocol marketing itself as a "cortisol blocker" is selling you something that, if it worked as advertised, would make you genuinely ill.


Now, what about the adaptogens and "adrenal support" supplements that are everywhere right now?


Ashwagandha has the strongest clinical evidence of anything in this category. A systematic review pooling nine clinical trials in stressed adults found that ashwagandha supplementation reduced cortisol by roughly 11 to 33% over periods ranging from 30 to 112 days, with no serious short-term adverse effects reported. The mechanism appears to be modulation of the HPA axis — dampening overreaction to stressors rather than suppressing cortisol entirely. The dose matters: most brands put ashwagandha on their labels at 100mg to 300mg, while the studies showing a 27.9% cortisol reduction used 600mg. You are reading "ashwagandha" on the label. You are not getting the clinical evidence the label implies. If you're going to try it, look for a standardized KSM-66 or Sensoril extract at 300–600mg daily for at least 8 weeks.


Phosphatidylserine has decent evidence specifically for blunting acute cortisol spikes — particularly those triggered by exercise. Clinical trials show that 600 to 800 mg per day can significantly blunt cortisol spikes after moderate exercise, with one study finding a 35% reduction in cortisol area under the curve. The caveat: the research is mostly in athletes or acute stress contexts. Evidence for chronic HPA dysregulation in everyday stressed women is thinner.


Everything else — the rhodiola, the holy basil, the "adrenal cocktails," the cortisol-support blends with proprietary formulas — either has weak data, mixed results, or evidence that comes primarily from animal studies. If someone is selling you a "cortisol support" formula built primarily around these ingredients, the marketing is ahead of the science.


Real talk: Supplements at best take the edge off an overactive stress response. They do not fix the root cause of cortisol dysregulation if the root cause is chronic sleep deprivation, a stressful job you've been in for five years, or a lifestyle that never gives your nervous system a chance to downregulate. No supplement bridges that gap. The lifestyle interventions come first.



Three Free Fixes That Actually Move the Needle


From the carousel — and from the research behind it:


Morning light within 10 minutes of waking. This is the most underrated and most evidence-backed thing you can do for cortisol regulation. Natural light exposure in the morning directly supports the Cortisol Awakening Response, anchoring the entire daily rhythm at its source. You don't need a fancy light therapy lamp (though those work too). Walk outside. Sit by a window. Five to ten minutes is enough to signal to your circadian clock that the day has started, which helps the downstream cortisol pattern fall into its correct shape.[²]


No caffeine for the first 90 minutes after waking. I know. I know. But here's the biology: during the CAR window, your cortisol is already doing its job of waking you up. Hitting caffeine in that window doesn't boost the effect — it spikes cortisol further on top of a peak that's already occurring, which contributes to the afternoon crash and the evening wired state. Limiting caffeine after 2pm, as it can interfere with evening cortisol decline, is a separate but related intervention. Delay your first coffee by 90 minutes and drink your last one earlier. This is annoying to implement and genuinely worth it.


A hard stop on screens at 9pm. Blue light from screens suppresses melatonin and keeps cortisol elevated when it should be in its natural evening decline.[³] The wired-at-10pm pattern that so many of us live in is partly a consequence of telling our brains, via screen light, that it's still daytime when our cortisol should be dropping. A 9pm cutoff gives your system a two-hour runway to begin the downslope before sleep.


None of these cost anything. All of them are supported by actual research. None of them are as exciting as a supplement stack, which is almost certainly why they don't get the clicks.



The Bigger Picture


Dr. Sara Gottfried — Harvard-trained OB-GYN and the author of The Hormone Cure — summarizes it simply: "Cortisol follows a rhythm. Disrupt the rhythm, disrupt everything."


That's the framing I want you to take away from this. The symptoms associated with cortisol dysregulation — exhaustion, mood instability, metabolic chaos, sleep issues — are downstream effects of a rhythm that's gotten knocked out of sync by a lifestyle that was never designed to support it.


It is a biology problem. And the biology problem has boring, evidence-based solutions that don't require you to spend money on anything you can't pronounce.
